New display showing up

Never seem anything like that but Im willing to bet that's its your TV and not the dish. You can test by connecting a DVD player or game console to the sane input the dish is on and see if it continues.

We just now saw that funny display while watching a recording on our Joey 4. It popped up for about a couple seconds. Wife says she has seen it bunches of times on our Hopper 3 Plus all last week. Hopper was then on version H382. It's now on H383.

I saw this on my friends TV when my wife was watching HGTV program at his house. He has DISH. I have never seen that before. What are accessibility controls?

If so, does it happen in the same spot in the recording if you play it back?
A test if it's the video output of the Hopper or in the input decoder.
