Do multiswitches fail?

I had dish install a dp34 to add 148 to my setup about 2 or 3 weeks ago and now my 921 is telling me that it "lost lock of sat 110 transponder..." So i did a check switch and it show test 1 of 3 on sat input 1, but (here's the weird part) shows test 1 of 28 on sat input 2!!! So i unhooked both inputs and did another check switch. then SWITCHED the inputs and hooked them up again and did another check switch and it show test 1 of 28 on input 1 and 1 0f 3 on input 2. So is the multiswtich fried or something? shouldn't it last longer than this?

IS there any way the 921 is really the problem?
 
When you swapped the lines on the back of the 921 the problem followed, so the problem is not with the 921. If you can get to the switch, swap the two lines there as well. Run a check switch again and if the problem is on the same port as before then you have a bad connection somewhere in that line, as in a bad fitting, barrel, or ground block. If the problem moves to the other port after swapping lines at the switch then the switch may be bad, but my money is on a connection somewhere. check all connections, make sure the copper center conductor in the fittings are not below the top level of the fitting, and make sure no metal flakes or bits of wire from the coax shielding are touching that center conductor.
 
swapped the line to another port and check switch showed up fine. test 3 0f 3 on both inputs on 921 (my 510 has been fine on another port).
So port 1 failed, are the other likely to follow soon or just leave it and run it as is??

Port 1 on my DP34 failed. I was only using 2 ports so I switched to another port. I contacted Dish Customer Service. Since it was only 3 months old at the time, they sent out a new switch. I had to return the old one or get hit with a huge fee for the switch.
