My on-screen Caller ID is enabled but has ceased to work.
Is there any way to re-set it so that it will display incoming calls?

In addition to the reset suggested by KAB -- which is unplug power from AC outlet for about 30 secs then re-plug -- you may want to do this:

-Press MENU
-System Setup - 6
-Diagnostics - 3
-Then try "connection" first. If it tests okay, you should be good to go. If it states that no phone line is detected, then click "send status" and let it go through its motions.

I honestly don't know the "rhyme or reason" for why this "wakes up" caller ID if you already have it enabled. On my receiver 311, I typically go through this re-set process to jump-start Caller ID about once every month or two. On my 625, the caller ID works pretty flawlessly. One thing you may want to check...make sure your incoming phone line is grounded which should be near your electric meter. I repaired a bad ground connection a few years back and the caller ID became more reliable thereafter. Make sure your incoming DISH cables and dish are grounded, too.

Mine stopped working a long time ago.
I'll have to try this and see if it works.
 
Thanks for sharing. The OP said that his is enabled and is not working. What you offer doesn't even come close to answering his question.

No luck with re-connecting the phone cord.
Replacing it would be waaayyy too much trouble...(for me at least!).
Ramen's "reversing the polarity" idea sounds like a good idea but I think he was being facetious.
Caller ID doesn't work on either of my two 211's, so I just going to wait until August when I switch one to a 612 and try to get resolved then.

Like I said, caller id doesn't work on either of my two 211's, so I'll just wait a bit rather than having to talk to the Dish people more than once (I hope!).

Does Caller ID work on your phone?
I was thinking that maybe the phone co turned it off by accident.
 
The caller ID feature of dish receivers is entirely too sensitive. The smallest thing will make it stop working for some reason (probably a really cheap modem). The reversing the polarity was probably not a joke. I got a tester for this a few moths ago, and probably 50% of the phone lines I test are hooked up backwards at some point. This has no effect on normal telephone operation, but when dealing with sensitive equipment it can be the difference between working and not working. Double check your DSL filters (if you have them). Any other solutions get fairly in depth (such as cutting lines that go nowhere in the customer access and the already suggested re-running of the line (preferably straight from the customer side of the box outside). You could also try disconnecting all your telephones (and DSL if you have it), then check and see if it's working. If it does, re-plug in devices one at a time testing it with each device to determine if one is making a significant amount of noise on your line.
