Johan, I just don't see anything unique in those photos that would help me to identify the maker of that actuator. I'm no expert and most actuators look about the same to me. If you can find a tag or some other marking, that would help in the ID. Sorry.
Can anyone else glean anything from Johan's photos?

If you look closely, can you find any numbers or anything stamped into the metal, visible through the paint?

I believe I have found the drive (again) after a long search. I didn't know I still had it. I'll have to look for the rest of the housing tomorrow.
