When I scan a satellite on my Fortec Mercury II receiver (just for the FTA channels) and it starts displaying the frequency, polarity and symbol rate, what does it mean when a [+] appears after the symbol rate?

I know on the Classic white TP's are new and blue are existing. So maybe the Mercury II dioes it that way (+ for new TP's)

If the scan finds new transponders, that doesn't necessarily mean that there will pop up new channels?

correct
They could be data channels or if you say "FTA only" they could be scrambled.

Also, I've noticed that a power/blind/smart scan will pick up a weak transponder (say with a 15 or 20 quality) but when it tries to scan in the channel there isn't enough signal strength to bring in a channel.

I did set my preferences to blind scan just for the FTA channels. I was thinking that possibly that [+] thing may have been related to 4:2:2 channels.
