I am returning one of my 622. Dish sent me the box but on the label there is no place for me to put my name, address or return authorization number (which I do not even have). For you guys that have sent back receivers, do I need a RA#. The label they sent me to put on the package only has where the package is going.

The label should have your address already printed on it - in very small type at the top somewhere .... I never had an RA#
I sent back two recently - on each occasion i printed out a sheet of paper with my name - account number - contents of the box i sent back (i.e. 622 *1 - remote * 2 - LNB *2) - Date i shipped the box - and the tracking number from the label.
I printed out two copies - one went in the box - the other went into my filing.
Probably overkill .... but both recievers were there and taken off my account within 5-7 days.
