Info on Dish/Starz promo

I signed up for Starz with Dish months ago because they were running a promo. You would get a $50.00 Visa gift certificate if you keep Starz for at least 3 months. The problem is that I cant find the info to send my bills to so I can get the certificate. I lost the postcard they sent me & there is nothing on the website. Does anyone out there have a link or info on this promotion?

Nothing on Dish's site. If you are sure it was 50, you might have to give dish a call or send starz an email.

It says copies. must be postmarked by 11/1/04 and received by 11/8/04
