Set AdBlock to disable on all pages on:

Disabling AdBlock on BOTH domains was necessary.
After AdBlock was setup, I removed the cookies for
dishanywhere.com
and
dishnetwork.com
Quit Safari.
I can now log in and out of DishAnywhere.com
