Just go an email from Dish asking me to complete a customer satisfaction survey. They ask one question as to whether you would recommend Dish to your friends. Then a second area to comment on whether you liked their service or not. I filled in my pros and cons. and clicked next expecting more questions. Instead, I got a Thanks for completing the survey. Wow! A one question survey. How thorough can they get.

"Treats" always help garner responses. How 'bout a free PPV cert, yo?
 
My guess would be that they may plan on follow up surveys depending on the answers. They may take a list of most frequently given responses for pros & cons and then put out another survey based on that information.
 
The way you answer your first few questions will decide if you see the real questions of not. If you don't fall in the demographic they are looking for then the survey will be done in one or two questions.

The big one is what do you do for work. If you check almost any of those boxes you will probably get kicked from the survey.
