Dish n' it up rebate form question?

I can't seem to find the rebate form they told me I need to send in to get my $200 worth of credit $100 each for my two 622 receivers.

Can anyone point me to the right rebate? I just got my new receivers last week. Also can I just send in one rebate form or do I need to use two?

Edit: To answer your question, looks like you can claim up to 2 receivers per form.
