Dish 500 dual lmb switch

To hook up a dish 500 with dual LMBs to a single reciever I need a SW21 switch right?

Yes. If you have a DishPro Dish 500 then you need a DishPro sw-21 switch. If you have a legacy Dish 500 (the ones where it does not say DishPro on the lnbf) then you need the legacy sw-21 switch that also does not say DishPro on it.
