Complete listing of available HD channels in the Pay TV market?

I'm looking at HD lineup of cable, D*, E*, and soon-to-be-defunct V* but I don't see a whole lot that each others are missing. So is it the broadcasters that are not pumping out these HD channels or are the pay TV providers just deciding not to carry them? Here's "my" list of HDTV channels available to the pay TV market. I'll exclude V*'s exclusive HD channels since they're not going to be available to other providers without sale of those rights.

Discovery HD Theater
ESPN HD
ESPN2 HD
HDNet
HDNet Movies
INHD
INHD2
NBA TV HD
RSN HD (Fox SportsNet, Comcast SportsNet, and etc.)

Universal HD (used to be Bravo HD)

Sorry but that's not enough HD for me. I think it's up to the programmers to bring out more HD channels. Are they waiting for the pay TV providers to increase capacity or what?

You forgot about SPICE HD, which is already on D*...
