Found the answer. Inside the box there is an installatin certificate. Buying from Costco is a much better deal due to their return policy.

I read a post from AVSForum that says you have a certificate with a Dish phone # valid for a free intallation if you already have Dish. I assume everything NECESSARY will be covered . For new customers the certificate includes even the dish (or dishes). I will search my nearby Costcos for a 921 tomorrow.
