If you loose the return kit but have an RA #, can you ship the receiver via FedEx?

You can ship the receivers however you want to back to Dish

You can also just ask them to send you the shipping label by email and you can use your own box.

I'm not so sure about "however you want." I would think that if it gets damaged due to improper packaging, Dish might have something to say about that...
