Rebate 942 to 622

Last week upgraded my leased 942 to a leased 622.

I see a lot of messages that I should get 10x10 for subscribing to HD package and an additional $100 for returning 942.

I can see a rebate form for 10x10 for HD upgrade on Dish website but how do I get $100 for returning 942. My questions:

1. Has the $100 rebate for returning 942 expired?
2. Do I need to send a form with my 942 return?
3. Where do I get the form?
4. Would rebate automatically appear on my account once they receive 942?
5. Is this also one of those deals where I need to approach the gods in the ceo office?

Its just that I do not see any reference on the dish website re $100 euipment rebate for 942 to 622 yet the board has so many messages that indicate such a rebate exists.

2. I'm not sure, but would try that unless address for rebate is different than shipping address for returning the 942.
3. It is on the Dish Network website.
4. It may take a month to process the rebate form, you don't send the form, you get no rebate.

But that's the problem. I have looked all over the dish website for the form but perhaps not at the right place. Could you please drop me a hint where I would find the rebate form for equipement return on the dish website?

Sorry, the rebate form is for new customers, there is no form for the return of the 942. You have to request details for return when you take the receiver off the account.

The avg price for a 942 on e-bay is about $250-300. I didn't want to change to the 622 so I looked for a used 942 for 2 months before caving and ordering the 622 to replace my 811. I'll keep my 811 instead of the $25 credit just to keep my original 4000 company in the attic.
