Link for rebate form?

When I called to upgrade my receiver to a 722, the CSR said the installer should leave the $100 rebate form, if he didn't I could download it from the website. Of course, the installer forgot to leave it and I forgot to ask. Now I can't find the rebate form on the website. Does anyone have a link? Thanks!!

Also this promo ended at the end of Sept., so not sure if this is the one or not.

EDIT: found the newer rebate form that expires in January 08.
