What is the process for getting the $50 rebates for the 622 and the 211 when installed via Dishin It Up?

When installed, they will give you a rebate form which you send in WITH a copy of the first bill you receive showing that you have signed up for HD programming (at least that's the proceedure for the 622).

isn't that for the 10 $10 credits for HD programming? is it the same process for the $50 per receiver credit when DIU? The installer didn't leave a rebate form so I guess I can just print it from the website.
