Just a quick shout-out for Dish Anywhere, recently spent some time in hospital, took tablet and because of good wifi, was able to watch my dish sub from my hospital bed. Also use it with tablet at home just about everyday.

Off to the right side of the picture you should see a four corner icon, you may have to touch your screen to get it to come up. Tap that and you should go to full screen.
Since an iPad isn’t a 16x9 screen you wouldn’t want it to.
The iPad screen is a strange shaped screen, its not 4x3 nor 16x9. What you are seeing is the actual shape of the real video size, and its not being stretched out to fill the screen on the iPad.
